A trailblazing Physician Associate whose rich multicultural background and expansive clinical journey are matched only by his passion for education, advocacy, and healthcare reform. With roots grounded in a Puerto Rican and Brazilian-Italian upbringing, Marcos embodies resilience, adaptability, and an unshakable commitment to advancing the PA profession—not only in Michigan, where he spent most of his career—but now back home, where he hopes to influence change across Puerto Rico.

Q&A with Marcos:

Can you share a bit about your background and what inspired you to become a PA?
My journey began as the first-born to a Puerto Rican father and a Brazilian mother of Italian descent, both in pursuit of the American dream. I was always drawn to healthcare—first considering pharmacy or medical technology—but my time working as a ‘Pharmacist Assistant’ at a nonprofit Catholic geriatric hospital opened my eyes to patient care. Discovering the PA profession at a time when it was still under-recognized. It was instead a leap of faith, but one I never regretted.

What career moments are you most proud of?
Each role across specialties taught me something new, but one defining moment was performing a post-mortem cadaveric cardiectomy for organ harvesting early in my career. Being entrusted with such a significant procedure shaped my respect for the responsibility we carry as PAs.

How has your role evolved, and what keeps you inspired?
I've adapted over the years, always welcoming growth like a chameleon. What fuels me is knowing I can make a real impact and never stop learning. Every chapter, every patient, every challenge has made this journey worthwhile.

What advice would you offer to PA students or new grads?
Striking a work/life balance is tough, but contributing to the profession gives it meaning. Be proud advocates of our PA brand, seek out passions beyond clinical work, and volunteer. It widens your perspective and keeps your professional fire lit. As Einstein said, “Try not to be a person of success, but rather a person of value.”

What’s your vision for the future of the PA profession in Puerto Rico?
We must stay resilient in the face of political barriers. Ley #71 has deeply impacted our ability to practice meaningfully. It must be amended or repealed. We PAs must educate, advocate, and prove our worth. Puerto Rico deserves access to the full capabilities of PA-provided care.

What changes would you like to see in Puerto Rico’s healthcare system
Improving access—especially in underserved communities—is crucial. PAs can bridge this gap by offering primary and specialty care, while also advocating for policies that let us practice at the top of our license.

How is the profession evolving on the island, and what’s your role in that growth?
It’s moving too slowly. Ironically, we’ve exported the PA model to 15 countries, but face resistance at home. The recent PA program closure is troubling, but I remain active—whether through published commentaries or community education. I’m ready to partner with like-minded advocates to move the needle forward.

Is there anything else you'd like readers to know about you?
I’m a lifelong learner and self-described “juggler” of passions—traveling, reading nonfiction, teaching, and public speaking. I’m even considering writing a guidebook on becoming a medico-legal consultant. For me, staying active and engaged is what keeps me centered and motivated.

Marcos was raised in Carolina, Puerto Rico and is bilingual in English and Spanish, with reading knowledge in Portuguese. He believes in living a life of value and is a firm believer that the PA profession holds the key to transforming healthcare access across Puerto Rico.
